How it’s done
If you are already an established patient of EMMC, we are ready to transition you to the Online Diabetes & Metabolic Management Program. We have an established electronic medical record detailing your treatment, labs, procedures, tests and diagnosis history. We use Skype or Webex for these appointments because it is free and successfully tested on both PC and Mac platforms. If your new to EMMC, our helpful staff will coordinate the collection of your health data, consult with your personal physician about his/her role and review the benefits of the program.
If you live outside of California we will work with you and your Primary Care Physician, just as if you came to us for traditional specialty care. Before we can get started it is required we have these two items signed and in your chart:
Once those are on file, EMMC acquires your medical records, current treatment plan, and list of medications. We have an initial online assessment interaction with you and from that we adjust your plan as needed. Like any in-clinic consultation, all changes to your treatment will be discussed with your Primary Care Physician. Likewise, you will need to visit your physician to check blood pressure, do brief physical exams and prescribe medicine and lab work.
Price
The Online Diabetes and Metabolic Management Program is being offered at $200 for each month of participation. Alternatively, you can pre-pay for a year of the program for $2,200 – a $200 savings.
As this is a global fee, it is not eligible to be reimbursed by your insurance company. All other aspects of your total health care are not included in this subscription pricing and will be assessed by your primary doctor or EMMC if Dr Joe is your primary doctor. Care is limited to Diabetes or Metabolic Syndrome and it’s complications, which include Retinopathy, Peripheral Neuropathy, Autonomic Neuropathy, Kidney, Cardiovascular, Hypertension, Peripheral vascular disease, Skin Infections.
