Diabetes can affect many parts of the body and can lead to serious complications such as blindness, kidney damage, and lower-limb amputations.

Working together, people with diabetes, their support network, and their health care providers can reduce the occurrence of these and other diabetes complications by controlling the levels of blood glucose, blood pressure, and blood lipids and by receiving other preventive care practices in a timely manner.

Thank you for taking the time to review the Diabetes Care Program. Diabetes 1 and Diabetes 2 is increasing in the United States. We now know how diabetes develops and how to interfere with it's progress. New evidence has been published that suggests reasonable steps can be taken to delay the onset of diabetes complications and studies are using the word "prevention". Complications can occur as early as seven years before the diagnosis of diabetes. With that in mind, we have available a series of tests that will assess your risk so you can focus on proactive care.

The Diabetes Care Program offers you the treatment, support and education you need to gain better understanding and control of your diabetes.

How it is done

Sign up for the Diabetes Care Program from Endocrine Metabolic Medical Center (EMMC) and enjoy unlimited office visits, major testing (CVProfiler, Peripheral Neurometer, Cardiac Neurometer, EKG/ECG) and treatment for complications using EMMC based equipment. We suggest you set up an minimum of one appointment per quarter and take care of the HbA1c test and one of the four allowed tests at that time.

This is an in-clinic program that does not take advantage of telemedicine and requires you be an established patient of EMMC. The Diabetes Care Program offers:

  • Unlimited office visits for one year starting from the day you sign-up
  • 1 CardioVascular Profilor test annually – examines the state of your cardiovascular health
  • 1 EKG/ECG test annually – shows heart health status
  • 1 Cardiac Neurometer test annually – tests for autonomic nerve damage
  • 1 Peripheral Neurometer test annually – tests for peripheral nerve damage
  • 4 Hba1c blood tests (1 each quarter) – reveals average blood glucose level over 3 month period of time

  • Medication discounts as can be obtained
  • Coupons for trials of new medications as appropriate

How much does this cost?

The Diabetes Care Program costs $2,500 for an annual membership, or you can join the monthly membership for $212.50. As this is a global fee, it is not eligible to be reimbursed by your insurance company.
